Follow these instructions to file an insurance claim for your medical wig / cranial hair prosthesis:
Step 1: Get Pre-Approved
Fill out the custom order form above to receive your cost.
Then ask your insurance carrier or FSA/HSA Provider how much they will reimburse you for your medical hair prosthesis, and how long it will take to get reimbursed.
Step 2: Purchase a Wig
Schedule a phone consultation if you need additional help. We will listen to your hair loss concerns, and answer all your questions. Each wig is handmade with the custom options you select. Simply let us know your hair needs and expectations.
Our goal is help you look and feel like yourself again. Then when you are ready, you can purchase a wig. You don't have to leave your home or hospital.
Step 3: File a Claim
Put in a claim with FSA/HSA.

For insurance download and send Claim Form #1500 to your insurance company (contact your provider if the form is not listed below). Other items you will need include:
1. Our Federal Tax ID [Provided upon request] Which will be included on our Medical Bill of Sale.
2. Procedure Code from your insurance provider. Commonly used procedure codes: Synthetic Wig (A9282 or 99199); Human (L8499)
3. Receipt paid in full as proof of payment
4. Rx with your diagnosis code from your doctor (It must be written for a medical or cranial hair prosthesis)
